Breakdown Expense Declaration Form

This form is used for the reimbursement of your breakdown or repair expenses covered under the Sİ-ASSİST road assistance package.

Please fill out the form completely and accurately.

Applications submitted without the required documents (payment receipts, invoice, etc.) will not be processed.

Your breakdown expense declaration will be reviewed by the relevant department after the verification and approval process, and if approved, reimbursement will be made within the specified period.

Note:
  • You can benefit from the package up to 3 times per year.
  • Refunds apply only to documented expenses.
